🦠 Chikungunya Re-Emerges in Cuba: What’s Going On?

The Caribbean island of Cuba is once again under epidemiological surveillance following the reappearance of chikungunya. Although this arboviral disease is well known, it had remained relatively controlled until recently. Below are the key facts you should know.

🔍 What We Know So Far

According to the Pan American Health Organization (PAHO/WHO), in its August 2025 bulletin, between epidemiological weeks 1 and 33 of 2025, eight laboratory-confirmed cases of chikungunya were reported in Cuba, all from Matanzas province.
The circulating genotype identified is the ECSA lineage (East/Central/South African) of the chikungunya virus.


Local media have reported a confirmed outbreak in July 2025 in the town of Perico (Matanzas), with typical symptoms (high fever, severe joint pain) and the activation of a contingency plan.

Travel-health authorities, including the U.S. Centers for Disease Control and Prevention (CDC), have already issued a Level 2 travel alert for Cuba due to chikungunya. Even the Russian embassy in Havana has advised its tourists to take precautions.

🌴 Factors Driving the Expansion

  • Favorable climate: High temperatures and the rainy season create abundant mosquito breeding sites—especially for Aedes aegypti, the main vector. Cuban media note that transmission peaks during “hot and rainy months.”

  • Population vulnerability: After years with little or no circulation, part of the population may lack immunity to the virus, facilitating its re-introduction and spread.

  • Co-circulation with other arboviruses: Cuba is also facing increases in dengue and Oropouche fever, complicating differential diagnosis and integrated vector control.

 

⚠️ Key Risks and Why a Country Profile Matters

Although the number of reported cases remains relatively low, the presence of the virus and the activation of local transmission foci call for reinforced surveillance.
The co-circulation of the ECSA lineage adds further risk, as this strain has shown strong epidemic potential in other regions.

For travelers and neighboring countries, it’s important to note that the outbreak has already triggered international alerts—destinations receiving visitors from Cuba should remain vigilant.

In terms of prevention, this situation highlights the importance of:

  • Strengthened entomological surveillance and elimination of urban breeding sites,

  • Rapid detection and notification of suspected cases,

  • Promotion of personal protective measures for residents and travelers (repellents, long-sleeved clothing, mosquito nets, screened housing).

Since early data point to a localized outbreak in Matanzas, monitoring is crucial to detect any potential spread to other provinces if vector control falters.

📈 What Does This Mean for the Region?

The expansion of the ECSA lineage, combined with the simultaneous circulation of dengue and chikungunya, underscores that vector control remains the weakest link.
The immediate challenge is to ensure early detection, prevent urban transmission, and strengthen community education on prevention.

At a global level, Cuba remains at a moderate-to-high risk, with active hotspots and climatic conditions favorable to the vector. Continuous monitoring, regional cooperation, and open information sharing will be essential to contain its spread.
